TELEHEALTH CONSENT TERMS
Introduction
Nature of the consultation This appointment takes place remotely via video or telephone. This differs from an in-person consultation and there are inherent limitations, including the inability to conduct a physical examination.
Your right to in-person care You may decline a remote consultation at any time and request an in-person appointment instead. Where clinically necessary, we may recommend you attend in person.
Clinical discretion If the clinician determines that a remote consultation is not safe or sufficient for your clinical needs, they will advise you to seek in-person care.
Privacy and technology Your consultation is conducted via a secure, encrypted platform. You are responsible for ensuring you are in a private, secure environment during the session. The same duty of confidentiality applies as in a face-to-face consultation.
Medical records Your consent to this remote consultation, its date, time, medium used, and clinical outcomes will be documented in your medical record in accordance with applicable regulations.
Data processing Your personal and health data is processed in accordance with our Privacy Policy, in compliance with EU GDPR, UK GDPR, and LOPDGDD where applicable. This consent is provided in accordance with Law 41/2002 on Patient Autonomy (Spain) and GMC Good Medical Practice guidance (UK).
